New Patients
We are always excited to meet new patients. Our goal is to make you feel as comfortable as possible. Every patient is unique, and we are committed to providing you with the highest quality care based on your individual needs. Above all else, we want to make your visit to our office a pleasant experience.
Please remember to bring the following to your appointment:
- Your medical and dental insurance card
- Your surgical referral slip
- Any recent x-rays
- List of Medications
Patients under the age of 18 must be accompanied by a parent or legal guardian. The first visit is essentially the collection of data that will lead to a diagnosis and discussion with you about the findings. Our doctors will review the ideal treatment option(s), along with a thorough discussion about benefits, risks and alternatives. We will also explain the procedures involved and the treatment sequence.
Less complicated procedures that do not require medical clearance, such as a simple or surgical extraction, can be performed at the initial visit.

Patient Registration
You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
Note to Patient: Upon filling out your first name, last name, email and at least one phone number field (a valid home or cell phone) a button will appear in the upper right corner titled "Save and Exit" . This button allows you to save and exit your form to complete at a later date. The system will send a registration email to the email provided on the form, so that you may setup a login to go back and finalize your registration form online.
